Slate Belt Nursing & Rehabilitation Center is a leading provider of long-term skilled nursing care and short-term rehabilitation solutions. Our 126 bed facility offers a full continuum of services and care focused around each individual in today’s ever-changing healthcare environment.
Slate Belt Nursing & Rehabilitation Center has been serving it's community for over twenty years. We are conveniently located off of Routes 512 and 33. Our facility is Medicare and Medicaid certified.
