Welcome to Courtyard Gardens

Since 1924, The Middletown Home has provided nursing care services to the residents of Central Pennsylvania. A long tradition of offering excellence in clinical care and quality of life is at the heart of our commitment to residents and their families. Our licensed staff of physicians, nurses, and certified nursing assistants provides residents with the highest quality nursing and medical care 24 hours a day.

Courtyard Gardens offers two levels of nursing care: intermediate care and skilled care. Intermediate care is available for residents who require assistance with daily living skills, including bathing, dressing, toileting, incontinence management, mobility, and/or feeding. Skilled nursing care includes rehabilitation care, wound treatment, respiratory therapy, and nutritional support. We are Medicare and Medicaid certified.

In addition, we provide a full staff of rehabilitation specialists, social worker, a registered dietitian, professional food service staff, and a large recreation and activities staff to implement an individual plan of care for each resident. On-site services include laboratory, x-ray, podiatry, mental health counseling, physical, occupational, and speech therapy, hairdressing, pastoral visits, and pet therapy. Resources and transportation are provided for dental care, vision care, and dialysis services.

Our 102-bed secure facility offers private and semi-private accommodations which include a shared bath. Residents are offered three nutritious meals daily in our large and spacious dining room, and for those who need assistance with eating, we have a full staff to provide for their needs. Our activity room is the hub of social activity with scheduled entertainment, crafts, holiday celebrations, exercise groups, special programs, and trips.
