Autumn Grove Care Center is a 103 bed skilled nursing facility located in Northern Butler County. The facility is fully licensed by the state of Pennsylvania, and is Medicare/Medicaid/HMO approved. The Administrator and Assistant Administrator have also obtained their Nursing Home Administrator's license from the state.
The staff and management of Autumn Grove Care Center believe that each resident is an individual. Each person should be able to live with dignity while maintaining his or her ability to function as independently as possible.
Autumn Grove Care Center offers an "Open Inclusion Environment" that allows residents with dementia to participate in activities and enjoy all the services offered while still securing their safety and providing needed structure and support.
We uphold a high quality of care by maintaining high staffing levels, ongoing training, continual availability of management staff, individualized services and care plans, and much, much more.
We accept long-term stays, short-term stays and respite stay residents.
SERVICES
Wound Care--24 hour daily nursing care with trained staff to provide care for decubitus, statis ulcers and surgical wounds
Dialysis--24 hour daily nursing care for peritoneal dialysis, contracted services to support hem dialysis, laboratory services and a dietician to assist with maintaining stable levels
IV Therapy--24 hour daily care to initiate and provide necessary IV therapy
Enteral Feeding--24 hour daily nursing care to provide for naso-gastric or gastronomy tube feedings
Nutrition and Hydration Program--Daily monitoring of meal intake and fluid intake as a measure to prevent malnutrition and dehydration
Skin Care--provided to promote healthy skin care and prevent pressure sores
Hospice Services--contracted hospice services are available for residents and/or family members who desire to have comfort measures only
Physician Services--we have 24 hour daily contact with attending physicians to assure the optimal level of care for our patients
Additional Contracted Services--we have contracted services with a geriatric psychiatrist, a dentist, and optometrist, an audiologist and a podiatrist, all of whom come to our facility on a regular basis
Dietary Services--menu choices are offered daily. All meals are planned by a licensed dietician and prepared by our own chef. We have a large dining room that is wheel chair accessible. We also offer a Therapeutic Dining area for residents requiring assistance with meals
Social Workers--we have two full-time social workers, including a behavioral specialist. These staff are available to provide ongoing support and to assure that residents’ and families needs are met
Activities--we have specially trained activity staff that provide a wide variety of activity programs to meet the needs of all residents. Activities are scheduled 7 days a week, including weekends and holidays
Housekeeping, Maintenance and Laundry--these services are provided daily as a measure of maintaining a safe and clean environment
Pastoral Care--local clergy provides visits and spiritual services
Financial Assistance--assistance is provided for special insurance claims and medical assistance applications
Beautician--a licensed beautician is available for on-site appointments
Security--entrances are securely locked to secure the safety and security of our residents
Pet Therapy—specially trained dogs (and their trainer) visit our facility regularly

The excellent quality of each rehabilitative service working with nursing staff make it possible to increase and maximize each resident's potential. Our therapists use a variety of specialized equipment to help our residents complete their specialized therapy program. After successfully completing a program specifically tailored to meet individual needs, many of our therapy patients have returned to their previous lifestyle.
