Set in beautiful Pike County, Pennsylvania, in the heart of Milford, Belle Reve Care Center is conveniently located to all of Northeast Pennsylvania as well as Orange County, New York, and Sussex County, New Jersey. Since 2001, we have been privileged to serve our senior residents with the best of care, service, amenities, and accommodations. Belle Reve Care Center offers our residents a continuum of care, with assisted living, Alzheimer's care, skilled nursing and rehabilitation services. Our experienced geriatric professionals and staff are committed to delivering quality care with compassion and respect.

The health care needs of older adults vary widely, and often their needs may change or increase over time. To meet the individual requirements of each resident in our assisted living community, there are four personalized levels of care, based on the need for assistance with activities of daily living.


ASSISTED LIVING PROGRAM

Belle Reve Care Center offers a comprehensive, highly regarded Assisted Living Program. Limited to 44 persons, we offer exceptional service, personalized care, beautiful surroundings, and the opportunity to form new and special friendships. Among the many benefits of living at Belle Reve are:

Peace of Mind

The Belle Reve Assisted Living Program combines luxury and practicality with peace of mind. Residents enjoy a safe and private environment, secure in the knowledge that there is a caring staff member always on call to help when needed. Our 24-hour call system and emergency pull cords can be used whenever a medical or service need arises. A dedicated, competent staff is on duty 24/7, and a nurse manager is always on call. If necessary, an attending physician is available to residents as well.

Spacious Accommodations

Belle Reve Care Center offers tastefully appointed, fully furnished Mission-style accommodations, with a choice of suites, one bedroom private, and semi-private rooms. Each room has a microwave, compact refrigerator, counter, sink and cabinet. A closet is provided for clothing, as well as a bureau or dresser and a comfortable chair. Residents may bring their own furnishings to personalize their selected accommodations. Our private bedrooms easily accept a king or queen size bed, as well as that favorite recliner.

All accommodations have individual thermostats for air conditioning and heating. Bed and bath linens are provided and are laundered weekly. Additional personal laundry services are available for a small monthly fee, but there is also a private resident laundry room available for residents who prefer to do their own laundry.

Restaurant Style Dining

Three delicious meals are served daily in our elegant restaurant style dining room. A salad bar, delicious homemade soups, and fresh fruit are available daily. Snacks and beverages are at hand as well. Our Dietitian and Food Services Director provides guidance on nutrition and special diets.


ALZHEIMER'S DISEASE/DEMENTIA MANAGEMENT PROGRAM

Belle Reve Care Center provides an Alzheimer's Disease/Dementia Management Program unique in the Northeast Pennsylvania area. The program is licensed as a Personal Care Unit by the Pennsylvania Department of Public Welfare. Limited to 19 residents, the program offers individualized programming for persons with Alzheimer's disease and other dementia-related illnesses. A dedicated and specially trained staff provides 24-hour care, with a nurse manager, attending physician, psychologist and geriatric psychiatrist available as needed. Our residents reside in a dedicated, secured area, offering safeguards against elopement.

Belle Reve provides our residents with a comforting, safe environment, allowing them to live together and participate in a regular schedule of group activities including games, arts and crafts, special events and recreational therapy. This approach is combined with daily set routines and meal times, affording a sense of stability. Pleasing music and old-time movies evoke pleasant memories of days gone by. Visiting hours for family and friends are unrestricted, and phone calls to our Social Services Director and staff regarding any special needs, issues or concerns are encouraged.

Belle Reve offers private and semi-private accommodations, including fully furnished, Mission-style accommodations, wall-to-wall carpeting, and beautifully decorated rooms. Three nutritious meals are provided daily, with regular and special diets available. Snacks, beverages and hydration programs are part of our daily service. Weekly housekeeping is provided, with bath and bed linens provided and laundered weekly. Enhanced care can also be provided to residents with special requirements, such as one-on-one companionship, coaching, personal hygiene, or incontinence issues.

Short Term Stays offer a family the opportunity to introduce their loved one to our Alzheimer's Program. This can be ideal if the caregiver needs time off, is going on vacation, or has other pressing personal, family or work obligations. SKILLED NURSING PROGRAM

Belle Reve provides both short and long term skilled nursing care. This highly regarded 30-bed unit offers safety and comfort as well as excellent medical care. The facility is licensed by the Pennsylvania Department of Health, supervised by physicians and administered by registered nurses on duty 24 hours a day. Additionally, there are licensed practical nurses and certified nursing assistants who provide the excellent care that has given Belle Reve Care Center our outstanding reputation in the tri-state area.

Short Term Care

For a patient who needs short-term rehabilitation or recuperation after a hospital stay, surgery, injury or illness, Belle Reve is the ideal place to regain strength and health in order to return home. For a resident of the Belle Reve Health Care Center, our skilled unit provides a familiar setting with friends nearby and 24-hour medical supervision. Discharge planning coordination by the Director of Social Services assures that the needs of the resident are met both during the resident's stay at Belle Reve and for his or her future health and well being. Care plan and family meetings are held to facilitate communication and arrange for post-discharge services.

Long Term Care

The skilled nursing facility staff prides itself on its dedication to the care, comfort and excellent medical attention to our long-term patients. Our dietician supervises the preparation of meals, and special menus are prepared and provided whenever needed. Both physical and cognitive recreational activities, such as exercises and bingo, are provided to encourage patients to remain as active as possible. On-site rehabilitation services are available for physical, occupational and speech therapy up to seven days a week.


REHABILITATION

Rehabilitation plays a key role in the treatment of patients with limitations resulting from injuries, surgery, or general medical conditions. Arthritis, hip fracture, knee or hip replacement, respiratory, cardiac, stroke and diabetic conditions may require rehabilitative services.

Our program has been designed to aid the resident in regaining and maximizing his or her individual level of independence. To this end, the resident performs moderate exercises and physical activity to ensure successful convalescence and a speedy discharge to home.

Whether post-surgical or general health issues bring a patient to the Belle Reve Health Care Center, the best of care will be provided. Physical, speech and occupational therapies are available up to six days a week. Results-oriented programming has given the Belle Reve Rehabilitation Program a truly outstanding reputation with positive outcomes year after year.

Referrals are made by hospitals and orthopedic and other medical doctors. All services require a physician's order. Because Belle Reve has a continuum of care, we are able to offer the residents of our Assisted Living and Alzheimer's Programs our in-house rehabilitation services. The three therapies Belle Reve offers are:

Physical Therapy - for ambulation, strengthening, transferring, pain management techniques, range of motion exercises, neurological rehabilitation.

Occupational Therapy - for positioning, fine motor coordination, hand rehabilitation. Patients learn to use walkers and extenders for greater independence upon returning home. Home environment assessment and environmental adaptation is available. Instruction is provided for daily living activities, such as bathing, dressing and homemaking.

Speech Therapy - for language disorders, cognitive and linguistic deficits, confusion and memory issues. Several swallowing and speech disorders are treated, including choking, coughing during meals, aspiration, difficulty speaking, improving slurred speech, and strengthening oral muscles.
