Bloomsburg Health Care Center is a 149 bed skilled nursing and rehabilitation facility conveniently located in Bloomsburg, Pennsylvania, just one block from The Bloomsburg Hospital. Bloomsburg Health Care Center is Medicare/Medicaid certified and provides short term and long term nursing care, as well as respite care. Each unit is fully staffed by licensed health care professionals to assure the utmost in quality care. Our skilled physical, occupational, and speech therapists can make remarkable changes in our Residents' quality of life following a debilitating illness, injury or surgery; with a high percentage of Residents returning back home to their loved ones. However, if a person's condition dictates continuing nursing care, we make sure each Resident receives the care they need in a loving and supportive environment.
Bloomsburg Health Care Center was established in 1983 and we have continued to grow with the changing times by adapting our services to the needs of the community. In 1999, we became affiliated with The Bloomsburg Hospital and renovated the entire facility; including creating a larger, more modern Therapy Department.
Bloomsburg Health Care Center offers a wide variety of services for our Residents to utilize, including on site x-ray and lab service, psychiatric and podiatry care. We have cable television throughout the facility and an in house beauty salon. We also provide laundry and mending services and also offer an area for our Residents who wish to do their own laundry.
Our Social Service Department serves as an advocate for your loved one as well as a support system for both families and friends of our Residents.
Our Dietary Department is diligent in providing individual food choices according to likes and dislikes. We provide specialized diets as prescribed by physicians, and according to medical needs.
Bloomsburg Health Care Center is currently in the process of redesigning our Third Floor secured unit, to better meet the needs of our Residents with Alzheimer's Disease or Dementia. We have formed several committees dedicated to researching and learning the best methods available to accomplish this project. Our focus is to provide a comforting atmosphere for our Residents with special needs, as well as needed support for their families and friends. Our plans are to teach specific staff members how to better care for those affected with these diseases, and to improve the environment on the Third Floor. Over the course of the next few months, we expect our project to be set in motion.
The decision to enter a skilled nursing facility is never an easy one to make. The staff at Bloomsburg Health Care Center will help you in any way to make the transition easier for everyone involved. We believe that the dignity and self worth of each individual is enhanced by enabling them to remain as independent for as long as possible.
ACTIVITIES
At Bloomsburg Health Care Center our Activity Department's main goal is to keep our Residents as involved as each individuals' physical and mental condition allows. We believe that a positive attitude is an important factor for good health and recovery. Therefore, our Activities Department has developed a wide variety of programs, providing positive motivation and stimulation. Among the Resident's favorites are Bingo, Pokeno, Bowl-A-Rama, and the ever-popular Spelling Bee. There are also many entertainment programs, provided by volunteers from the community, who bless us with their music and singing. We have monthly birthday parties, where all Residents are invited to celebrate all birthdays of the current month.
Our annual events include, Valentine's Day Party (with the crowning of a king and queen), Pumpkin Festival, Trick or Treat Night, Summer Carnival, New Year's Eve Party, and you can't forget our day trips to the Bloomsburg Fair. A new tradition was implemented this past Thanksgiving in which the Administrative Staff serves a formal dinner to the Residents.
The Resident Council meets on a monthly basis to discuss ideas and concerns to be addressed. Residents also receive a monthly newsletter which spotlights a different Resident each month. In the summer we have picnics at various parks, and year round we schedule trips to the local mall. Of course, family members are always invited to participatein any activity with their loved one.
Our Activity Staff makes sure the daily newspaper gets delivered; they read to, and write letters for our Residents and provide a listening ear and a hand of comfort.
The Activities Department is an integral part of each Resident's psychosocial health and contributes to the general well being of each Resident on a daily basis.
REHABILITATION
Bloomsburg Health Care Center offers several levels of Rehabilitation and Restorative Nursing Services to our Residents; Active Rehabilitation Therapy - Focuses on retraining, education and the teaching of skills following an acute injury, illness, or surgery. Rehabilitation is a task-oriented discipline with a specific aim to be achieved within a definite period of time. We provide in-house licensed Physical, Occupational and Speech Therapists. Therapy sessions are conducted five days a week, Monday through Friday. Duration and intensity of therapy sessions are individualized to each Resident's level of tolerance and capabilities. As goals are met, new goals are established until each Resident reaches his or her maximum level of functioning.
Restorative Nursing - Focuses on restoring or compensating for skills lost through disuse due to physical or mental changes. Restorative Nursing is based upon the nursing model with less continued direct input from formalized therapy. Restorative Nursing seeks to maximize and prolong abilities with specific, measurable objectives and is a continuing process. There are three levels of Restorative Nursing. Each level is based on the individual Resident's physical and mental capabilities and is reviewed and up-dated on a regular basis.
- Level 1 is conducted in the therapy room, utilizes therapy equipment, moist heat packs, etc. It also involves the Rehabilitation Nurse and Rehabilitation Certified Nurse Assistant.
- Level 2 is conducted on the unit by the Restorative Therapy Aides.The services provided at Levels 1 and 2 are those tasks delegated, taught and supervised by therapy and the restorative therapy measures that are more difficult to provide related to a Resident’s condition. For Level 1 and 2 services, the restorative staff serves as the middleman between Active Therapy and Basic Restorative Nursing provided by the regular Certified Nurse Assistants and Nurses.
- Level 3 involves the regular Certified Nurse Assistants and Nurses. Restorative Nursing Measures are provided to maintain a function, or attain a higher level of function, if provision of these measures are not complicated by the Resident's condition. Level 3 Restorative Nursing Measures include the following; range of motion, contracture care, positioning, ambulation, activities of daily living, safety device care, feeding skills, foley catheter care, bowel and bladder retraining, toileting programs, and gait belt usage.
