Evergreen Nursing Center is a Licensed Skilled Nursing Center and a Licensed Personal Care Facility, located in a beautiful country setting. There are 115 beds in the Skilled Nursing Facility and 26 beds in the Personal Care Facility. At Evergreen, we provide quality health care in a atmosphere that is warm, comfortable and reassuring. Our greatest asset is our staff, highly trained, friendly and eager to be of service. By balancing professionalism with warmth and caring, we encourage residents to reach their full potential. Resident accommodations are cheerful, colorful, and pleasant. We encourage a home like atmosphere and many residents personalize their rooms with possessions from home.
Evergreen Nursing Center offers 24 hour Skilled Nursing Care which includes the following:
- Intravenous Therapy
- Tube feedings
- Wound Care
- Oxygen Therapy
- Lab Testing
- Radiology
- Dialysis at an alternate location
- Special Diets
- Physical Therapy
- Occupational Therapy
- Speech Therapy
- Incontinence Management Program
- Individualized Care Planning
- Discharge Planning
We recognize the importance of resident and family involvement in all aspects of life in our facility. Our staff is always available to assist and answer questions. A full range of planned activities is scheduled on a daily basis. Crafts, musical programs, cooking, exercise programs, religious programs, movies, holiday celebrations, games, and picnics are among the activities that we offer.
We have a Personal Care Facility for those individuals who do not require the skilled level of care. Both private and semi private accommodations are available. If the time comes when Skilled Care is needed the transition is easy, residents just have to go through a connecting doorway. Likewise it is just as easy to go from Skilled Nursing to Personal Care. We are able to provide a continuum of care for those who need it.
